The nature of speech-language pathology practice in the acute care setting of large medical institutions is varied in its scope and is continually evolving. Traditionally, one of the roles of the speech-language pathologist in this setting has been to provide the medical team with valuable information that contributes to patient management. In recent years, clinicians have felt the pressure imposed by aggressive cost containment initiatives that result in shorter lengths of stay and mandate completion of a diagnostic work-up of speech, language, and cognitive disorders with sicker and medically unstable patients. Despite this change, our role as diagnosticians of communication disorders continues to be vital. The speech and language diagnoses provided by the speech-language pathologist may assist with lesion localization in patients with unclear neurologic diagnoses, may help steer medical management for the patient, and may influence discharge plans. Therefore, speech-language pathologists require informed diagnostic decision-making procedures that lead to accurate diagnosis of speech, language, and cognitive disorders resulting from underlying neurologic conditions.
